Referral & Insurance Info

Endocrinology Practice Referrals and Insurance

Florida Diabetes and Endocrine Center is a consultative practice.  It is important that you continue your relationship with your Primary Care Physician and/or previous Endocrinologist.  Please ask them to fax your Medical Records to us at (407) 303-2805.

If you are a member of an HMO plan you must secure an authorization from your primary care physician prior to each visit.  Please ask them to give you the authorization number so that you can be assured it has been taken care of before coming for each office visit.  If you have any questions, contact us at (407) 303-2801.

Education Referrals and Insurance

All Education Services require a physician referral in order for Florida Hospital to bill your insurance.  If you are interested in attending a diabetes education class or individual counseling session, please contact our office and our staff will assist you with the necessary referral and verifications.

Diabetes Education is offered as an outpatient service of Florida Hospital.  While most insurance plans and Medicare cover diabetes instruction, education is not covered by Medicaid.  The amount due from the patient varies by insurance company but is often as little as an office visit co-pay.  We ask that you be prepared to pay this amount at your first visit.  For more information, contact us at (407) 303-2822.
