About Our Practice

Location

The Florida Diabetes and Endocrine Center is located at Orange Avenue and Winter Park Street across from the Florida Hospital Cancer Institute.

Business Hours
Our business hours are Monday - Thursday, 7:30 a.m. to 4:30 p.m. and our phones are available on Friday, 8:00 a.m. - 12:00 p.m.

Appointments
Appointments are available Monday - Thursday, 7:30 a.m. - 5:30 p.m. To schedule an appointment,  please call (407) 303-2801 during business hours.

No Show Policy Because our office services are in such high demand, we usually have a waiting list to see our physicians. As a result, we have found it necessary to institute the following policy:

There will be a $25.00 charge payable at your next appointment if a scheduled appointment is cancelled less than 24 business hours prior to the appointment. On your third “No Show” we may find it necessary to discharge you from our practice.

Please note that we have voice mail 24 hours a day, and you can leave a message at your convenience at least 24 business hours prior to your appointment to cancel with no charge.

Financial Policies and Insurance and Information For routine office visits, payment is expected at the time of service. Payment may be made by cash, check, VISA or MasterCard. Our fees are usual and customary, and covered by most insurance plans. Covered claims will be filed by our office as a courtesy to our patients. If you wish to file your own claim, we will provide you with the necessary information to do so. Please note that although your insurance plan may cover a portion or all of your medical expenses, the patient is ultimately responsible for all charges incurred. Insurance companies vary tremendously in the amount they will cover.

Labs and Diagnostic studies Your laboratory and diagnostic studies will be performed at the preferred facility of your insurance carrier. You will be given a lab requisition at the time of your office visit. We ask your assistance in ensuring that all results are directed to our office.
